Child Find Referral Form
If your child is birth-5years old and you are concerned about his development (speech & language, physical development, cognition, self-help skills, psycho-social development etc.) ask your local school to initiate a "Child Find Referral Form" (like the one below). This will alert the school district and provide a tracking system so that your child's potential needs will be addressed through proper screenings.
